Application for National Entitlement Card (Disabled)

You must bring with you one document to confirm Proof of Address (dated within the last 3 months) and also one of the following from a category below that applies to you.

Disabled
  • The higher rate of mobility component of the Disability Living Allowance
  • The higher rate or middle rate of the care component of the Disability Living Allowance
  • Attendance Allowance Award Letter
  • An Award Letter of Confirmation from the Veterans Agency
  • Be in receipt of a Disabled Person's Parking Badge (Blue Badge)
Deaf
  • Confirmation from Audiology at Stirling Royal Infirmary
Epilepsy
  • Letter from DLA (dated within the last 12 months) confirming driving license refused or revoked
  • A valid NCT003 certificate of Eligibility verifying Epilepsy
Mental Health Issues
  • A valid NCT002 Certificate of Eligibility verifying mental health condition
Terminally Ill or Progressive Degenerative Disorder
  • DS 1500 Report
  • Hospice Admittance Letter
  • Letter from Hospital Consultant
Limb Loss
  • One or both lower limbs
  • One lower limb and one upper limb
  • Both upper limbs
Visually Impaired
  • Registered Blind - Registration Certificate
  • Partially Sighted - Registration Certificate
Companion (must fall into one of the following four categories)
  • Higher or Middle Rate of the Care Component of DLA
  • Attendance Allowance
  • Registered blind
  • Living in a Care/Residential Home or Hospital and who is eligible for the higher or Middle Rate of the Care Component of DLA or Attendance Allowance

An application will be completed by one of our staff, signed by you and you will also get your photograph taken. The completed application will then be sent away and your National Entitlement Card will be sent to you through the post within approximately 14 days.
