Application for National Entitlement Card (Disabled)

You must bring with you 1 document to confirm proof of address (dated within the last 3 months) and also one of the following from a category below that applies to you:

Disabled

Deaf

Epilepsy

Mental Health Issues

Terminally Ill or Progressive Degenerative Disorder

Limb Loss

Visually Impaired

Companion (must fall into one of the following four categories)

An application will be completed by one of our staff, signed by you and you will also get your photograph taken. The completed application will then be sent away and your National Entitlement Card will be sent to you through the post within approximately 14 days.
